At Pivot Physio, we will keep any personal information you provide confidential, and will not release it without your permission. Your information will not be sold, or listed for the purposes of advertisement, solicitation, or fundraising.

It is understood, within the realm of general patient care purposes, your personal information may be used in the context of: patient registration, to obtain medical records from former practitioners, in conversations with colleagues for opinion/care, insurance purposes, to pursue collection of unpaid bills, general research purposes, or for other reasons directly related to your care at Pivot Physio.

I authorize the clinic and its associated health professionals to collect my personal and medical information as documented above. In addition, I authorize the clinic and its associated health professionals to communicate with my family doctor and/or referring doctor as deemed necessary for my beneficial treatment. I also understand that my personal and medical information is confidential and will only be disclosed to third parties with my permission.
